With Medicare BlueSM Plus (PPO), you can enjoy all of the benefits that the original Medicare plan offers and more. Medicare Blue Plus (PPO) is open to all South Carolina residents who have Medicare Part A and Part B.
The monthly premium for Medicare Blue Plus (PPO) is $99 for Upstate and Midlands residents and $121 for Coastal and Rural residents. If you currently pay a premium for Medicare Part A and/or Medicare Part B, you must continue paying your premium in order to keep Medicare Part A and/or Medicare Part B and remain a member of this plan.

Vision care benefits are only available through opthalmologists, optometrists and opticians in the EyeMed network. On behalf of BlueCross, EyeMed provides a vision care provider network. You can find Medicare Blue Plus (PPO) dental providers within your ZIP code area using the Dental Provider Locator or search the Dental Provider Directory. You may use any of these dental plan providers for in-network benefits, and you are free to change providers at any time. Your dental care benefits are provided through our partnership with Avesis. On behalf of BlueCross, Avesis administers dental care benefits. Avesis is an independent company. Just show your BlueCross ID card when you visit an in-network dental care provider for services. If you choose to go to a dental care provider who is not in the network, you may be required to pay the provider at the time of service and file a claim with Avesis for reimbursement.
You can get a dental benefits claim form for out-of-network dental services by calling Avesis at 1-977-225-7460, Monday through Friday, 7 a.m. - 8 p.m. (TTY users call 1-800-735-8583).
You can switch plans during the open enrollment period every year from November 15 to December 31. Medicare may also allow you to change plans under special circumstances, such as:
