  • You can pick one home fitness kit per benefit year. 
  • You can visit the online library of resources for exclusive articles and tips for health eating.

Consumer Assessment of Health Care Providers and Systems 

This anonymous survey seeks patient feedback on their experience with the healthcare system, including how easy it was to obtain needed care, how well your doctor communicates and satisfaction with your health plan customer service. You may be contacted by phone or email to complete this annual survey. 

Health Outcome Survey

This patient-reported survey is conducted with Medicare beneficiaries to gain insights on their physical and mental health. You may be contacted by phone or mail to respond to a baseline survey and then receive a similar follow-up survey two years later.

BlueCross Essential, Rx Value and BlueCross Rx Plus are stand-alone prescription drug plans with a Medicare contract. Enrollment in BlueCross Total, BlueCross Total Value, BlueCross Blue Basic, BlueCross Secure, BlueCross Essential, BlueCross Rx Value or BlueCross Rx Plus depends on contract renewal.
